WHAT TO DO WHEN YOU ARE DIAGNOSED WITH CANCER

There are several different divisions of clinical immunology- one of them is ONCO-immunology (which is the treatment of cancer through the strengthening of the immune system). The type of patients I work with most are patients with severe and chronic diseases and are mostly cancer patients. Cancer by definition is an immune deficiency disease because if the immune system is functioning normally, it will protect us from getting cancer of any sort, regardless of what the stresses are. What happens for most people is as a result of things such as poor nutrition, infections, toxins, traumas, stresses, et cetera, in other words daily life here on Long Island, their body gets a little bit toxed-out, the immune system starts to dip in terms of its ability to surveil and protect us from abnormal cells.

Long Island has been identified throughout the media as a cluster for autoimmune diseases & autoimmune thyroid diseases, specifically Hashimoto's thyroiditis. There have been a number of studies that actually look at the relationship between thyroid disease and cancer. And the direct relationship comes from the fact that the thyroid hormone is critical in stimulating the activity of certain cells that protect us from these abnormal cells. Certain cells called 'natural killer cells' and cytotoxic lymphocytes require thyroid hormone in its unadulterated form, in other words, not being attacked by antibodies, and not being too deficient in order to activate and to protect us from these abnormal cells. So there's a direct connection between thyroid problems and cancer problems.

PRIORITY 1: WATER FILTRATION
I recommend for all my patients as a starting point that they get a water purifier and that they only drink either good quality bottled water, preferably in a glass bottle, or that they use a good filter. There are several good ones on the market, but you have to make sure to check the filter regularly, to make sure that it's still working adequately for your needs. All the water you drink should be either filtered or bottled. If it comes from the tap it has to be bathing, that's another story and that's a bigger problem, and for that you're gonna need a whole house filter. The first thing you need to do is to stop putting more toxins into your system. So you wanna have clean water. Clean water is critical from the point of view of detoxification and from the point of view of allowing the enzymes to get from point A to point B in as quickly a manner as they need to do.consists of chemicals that were dumped there years ago by the aerospace industry, Grumman and the like, when they were building aircraft here and the space shuttle here and the lunar lander here and all sorts of things, and they just dumped these chemicals into the aquifer. Unfortunately, at Long Island, we only have one aquifer for the entire island. So a news daily showed this plume underneath Bethpage and they showed lines around it like a bullseye showing where the plume would be in five years, in 10 years. And in 15 years from now, it's gonna be all the way out to Montauk Point.

GO ORGANIC!
So next on our list, once we have the water situation covered, is to look at the kind of food that you're eating. I recommend that you eat organic food as much as possible. Here on Long Island, we have lots of good sources for organic food. Out east, there are whole farms, that grow organic produce. Places like Whole Foods and Wild By Nature, which have good organic produce tend to be more expensive but they have usually a bigger selection. Also, if you're going to eat protein in the form of eggs, beef, pork, what have you, they should be organic as well. Take the extra initiative and a little Google-searching to find a good butcher in your area or health food stores. Your diet should be as organic as possible-- keep things as clean as possible!

STEM CELL TREATMENT PROCESS (part 2)

PRE-TREATMENT WITH SUPPLEMENTS
To maximize the effect of stem cell therapy, I first conduct some blood tests to identify the amount of inflammation by what the inflammation markers are in the person's blood. High markers (like a C-Reactive Protein produced in the liver which this level is high in the blood) affects the procedure by diffusing the stem cells quite evenly around the body because the CRP is all over the body. It draws the stem cells all over the place-- not allowing them to concentrate.  Hence, I recommend that if anyone has high inflammatory markers, I would first induce preliminary therapy to reduce the systemic levels of inflammation. This helps the inflammation that's concentrated around an area of damage to stand out from the background of what's going on biochemically so that when the stem cells are delivered, they'll know where to go. If you don't pre-treat and deal with the systemic inflammation, the stem cell treatments may bring far less effective results and much less quality impact than they otherwise might have. Once the stem cells are induced, there are certain supplements that help the stem cells to grow, mature and to replicate in order to do the things they need to do.

Stem cells have specific requirements- somewhat more demanding than other tissues. Because the metabolic rate is higher, the nutritional needs are higher and we're trying to keep the inflammation down so that it's not distracting them from their primary work that we're trying to have them do.  Certain supplements are available and are known to act as systemic anti-inflammatories (designed to help support the metabolism of stem cells).  These supplements assist in stimulating the various repair enzymes that DNA uses while maximizing the life and efficiency of the stem cells that have been re-implanted back into the targeted area.

Taking the supplements is very important in order to have the full results, impact and benefit of the stem-cell therapy. The supplements should be taken for at least a year because the stem cells can be active for that time period (or longer) repairing the damage once they're introduced into that area or migrate there through the blood.

TIME ELEMENT FOR THE PROCEDURE
This whole procedure is a fairly quick procedure. The procedure to harvest the stem cells takes about 15 or 20 minutes and it's done through two little punctures in the skin- one on each side of the abdomen. And then beyond that, it takes about an hour and a quarter or so to concentrate and purify the stem cells so they can be reintroduced either intravenously or intra-articularly into a joint to help repair a specific area of damage.



THE GLOBAL AGENDA FOR STEM CELL RESEARCH
Major stem-cell research is currently being conducted worldwide with tremendous results- seeking more ways of harvesting the stem cells and how to take the adult stem cell and reprogram it back into a pluripotent cell.  so they can use it to regrow new organs or regrow whatever damaged area of the body that the person has, and basically be able to make people more bionic and rejuvenate them with these new organs and tissues.


In the U.S., stem cell therapy is available in specific applications where the use of stem cells can be harvested from the bone marrow or from adipose tissue to help to repair joint damage. Common uses are recognized for arthritis and in the wear and tear that we have in our musculoskeletal system.  In other countries, major breakthroughs in Stem Cell technology are being conducted with significant clinical successes in many different areas helping to solve issues with severe and chronic diseases such as ALS (which is Lou Gehrig's Disease), Alzheimer's disease, diabetes, Type I diabetes and even rheumatoid arthritis as well as all sorts of autoimmune diseases.

FROM A SINGLE CELL TO THE CELLULAR REPAIR PROCESS (part 1)

We all start off as a single fertilized ovum deep in the uterus of "Mom". From that single cell then divides into two, into four, into eight and into the different stages of the embryological transformation, which ultimately winds up as a fetus, a newborn baby, and finally as an adult.  Along that way, we produce a variety of cells that are called pluripotent cells.  Pluripotent cells can transform themselves into basically any cell that makes up US.  Down the line, these cells start to differentiate into cell lines that then form the ectoderm, the mesoderm, etc., certain cell layers in our body which turn into various organs. These organs have a direct relationship with each other as we grow and mature into adults, but the underlying embryological connection still remains.

STEM CELL THERAPY has many different aspects depending upon which stem cells you're using. If you can go way back and use a pluripotent cell (which we can only get from an embryo), we then have the capacity to turn that cell into new organs, new cell lines, new white blood cells, new red blood cells, and to repair all sorts of diseases potentially that an adult may have.

If we're talking about adult stem cells (what you and I have in our body right now), these cells are already committed to a particular cell line and can only form certain kinds of tissues. So, I work with people using stem cell therapy by removing some of their stem cells that are attached to adipose connective tissue (connective tissue of fat that I will gently extract using surgical instruments) and from here we'll remove some of these stem cells. We will concentrate them, we will purify them, and then inject them into damaged joints where there's arthritis and severe degeneration and these stem cells can help to rebuild the damaged cartilage and help to repair a lot of that inflammatory damage. It won't rebuild the entire joint because the adult stem cells aren't powerful enough to do that but it can repair a lot of damage, relieve pain (enough to get people walking again much more normally) and enable the patient to exercise and get them back to their old self and "turn back the clock".

STEM CELL VS. INFLAMMATION
Stem cells can also be purified and injected intravenously as it is capable of migrating to an area of high inflammation. So if somebody has a lot of inflammation in their joints as with arthritis, the stem cells will travel and settle in that particular area and do the work of repairing tissue damage.  However, in cases of extreme inflammation in areas like the lungs from COPD, or in their kidneys from renal disease, or in their pancreas because of late stage diabetes per se, then the stem cells have been proven to migrate there and can help to support those tissues.


Unfortunately, the stem cells that come from adipose tissue are committed to a particular stem cell line and can only form certain kinds of tissue. So their ability to repair some of these organs is very limited, but still there is some repair that can be demonstrated and it can greatly improve a person's function of these systems.  Stem cells can give them a lot of relief from symptoms while helping to cut back on medications.  It also helps to have more energy- greatly improving their quality of life.

DIABETES 101

There are different types of diabetes. The type 1 where the person is generating antibodies against their pancreas at a very early age usually, and it could result in a very severe form of diabetes that's somewhat unstable. Later on in life, many people as a result of poor nutrition, not eating the right nutrients to support their biochemistry, having too much alcohol, having too much carbohydrates, too much sugar, basically burn out their poor pancreas and its islet cells and its ability to produce insulin, and before you know it, the blood sugar starts going up. There's hormonal things that can affect the production of insulin and the response as well, and this is often seen when some women are pregnant, they can have pregnancy-induced diabetes, which is usually a short-term thing that resolves after the pregnancy is over.

There are many factors that affect the functioning of our pancreas. The pancreas is a very fragile organ located on the underside of our stomach. It can become inflamed, people can get pancreatitis as a result of many different toxins that we could be exposed to. There are some toxins that are directly toxic to our pancreas that we find in our environment. There are other toxins, things that can attack our pancreas in terms of viruses, and yet there are other things that can attack the pancreas through such simple things as the excessive use of alcohol. You don't have to be a falling-down drunk to wind up with pancreatitis. You just have to be more sensitive to the alcohol than the guy sitting next to you, and everybody has a different threshold of sensitivity. It has to do with how our liver metabolizes the alcohol and how fast it can get rid of the waste products before they accumulate in our system and cause inflammation to some of these fragile organ structures. In any case, once the person has diabetes, they have a problem of their carbohydrate metabolism. Carbohydrates are things like grains, sugars. Alcohol is metabolized into various sugars and carbohydrates therefore.

Even fruits and vegetables are carbohydrates, although they're classified as complex carbohydrates. They're metabolized much slower and turn into sugars at a much slower pace, usually at a rate that our body can easily deal with. If somebody already has full-blown diabetes, and even the fruit sugar that we find in our fruits, whether it be berries or whether it be bananas or apples or pears, it may be more than the body can deal with and it can raise our blood sugar as well, so you have to be careful.

So, when trying to address a diabetic situation to help somebody out, first thing I do is take a look at some blood tests and see if they're making antibodies against their pancreatic islet cells or other enzymes, things that can contribute to the production of diabetes, which means that I have to take an immune approach to straighten things out. I look at various inflammatory markers because sugar is an inflammatory substance and can lead to the progression of other inflammatory-based illnesses, systemically, such as atherosclerotic heart disease, and so we take a look at the inflammatory markers and address that. And then, we take a close look at the person's life, lifestyle, and stress. Stress and its release of catecholamines, epinephrine, normetanephrine, et cetera, will cause an increased level of blood sugar, as can an excessive production of cortisol when the adrenals are under an excessive level of stress for a prolonged period of time.

When we take a look at the diet, what we want to see and what we want to help the person evolve towards is a diet that's high in proteins, higher in good fats, if you will, very low in simple carbohydrates. So, basically, Cheerios and two tablespoons of sugar in a cup of coffee doesn't work any more. And yes, I did say tablespoons because there are people out there who put sugar in their coffee by the tablespoon, not the teaspoon. Be that as it may, you have to pay very close attention to the amount of simple carbs in your diet as this will immediately raise blood sugar.

Going onto what's referred to as a ketogenic diet where the body is eating a lot of proteins and a lot of good fats can very quickly stabilize blood sugar and help to bring it down. There are different nutritional shakes that people can use that can help to stabilize their biochemistry and help reverse the immediate issues of diabetes and stabilize it. And it's very important to stabilize the diabetes because it can lead to cardiovascular heart disease, blindness, kidney damage, peripheral neuropathy, and so on and so forth, no fun. So you really wanna get the diabetes under control as quickly as possible, but that doesn't mean you have to give up all sorts of fun in your life and in your diet. There's plenty of good foods out there that can be eaten. It's a matter of being a little bit more creative with your diet, perhaps getting a good cook book. There are several good cook books out there on ketogenic recipes that I would highly recommend.

Lots of water is very, very important. The usual recommendation of eight 8-ounce glasses is a good place to start. Make sure it's very clean water, such as from a good source of bottled water or good filtered water. You don't want to be adding more toxins to your system. You want to be getting a good night's sleep because as I said earlier stress will make the diabetes worse. Exercise is very important because exercise will burn sugar. Exercise will lower your blood sugar almost immediately, so you want to get exercise on a regular basis. And I'm not talking about the New York City Marathon here, but 20 minutes of a brisk walk a day is enough to significantly impact and help your diabetes. Together, this sort of integrated strategy can significantly reverse the effects of diabetes, stabilize it, and help to ensure your possibility for a long and healthy life. It's important to talk to your physician about some of these things. Look at your diet. Look at what could be done with various medications in the short run that might help you get to your goal sooner because you want to minimize any potential side effects of the diabetes as quickly as possible.

CVID : COMMON VARIABLE IMMUNODEFICIENCY

One of the primary topics of the study of immunology is the area of IMMUNE DEFFICIENCY and the various types that exist. There are GENETIC immune deficiencies that a person is born with, and some of these can be quite severe. For example years ago, there was a case of David, the boy in a bubble, and he had a severe combined immune deficiency syndrome, SCIDS. And he wound up living in a environment that was very filtered air, and he couldn't eat anything that was essentially not sterilized first because he had no immune system to protect him from infection. And at one point they tried to treat him with a bone marrow transplant from his sister. The transplant was a success. She was a good match but unfortunately, unbeknownst to them and they didn't test for it back in those days, it was a number of years ago, she had a latent Epstein-Barr virus in her system, which when it got into his system because he had no immune system to protect him, reactivated and wound up killing the poor guy.

In any case, that's a severe case. Most people have a much milder form of immune deficiency called common variable immunodeficiency, CVID for short. Common variable immunodeficiency can be a very mild situation when you do blood tests for the different antibodies that protect us from infection, IgG and IgA subclasses. You'll see one or more subclasses running below the normal range. Each subclass has to protect us from different kinds of infectious critters. And when one of these is running below the normal range, it predisposes that person to recurrent colds, flu, sore throats, swollen glands, strep infections, virus infections, whatever it happens to be that that antibody is supposed to protect us from. Now, common variable immunodeficiency can occur for a number of reasons. Any of the things that cause damage to our immune system, poor nutrition, infections, toxins, traumas, stresses can all lead to a CVID situation.

Now the problem with CVID is manyfold. On the one hand, the person is suffering from recurrent illnesses. On the other hand because there is damage to a particular part of the immune system, it causes a shift in the immune system. Typically, it causes a shift such that the immune system shifts its resources into the pathway that has to try to repair those immune deficient molecules. That pathway is called a Th2 pathway. When the immune system shifts its resources into that pathway, it deprives resources from another major pathway of our immune system called the Th1 pathway. And when that pathway becomes then secondary deficient, it increases our risk to other sorts of severe and chronic diseases, perhaps the worse of those is cancer. So when somebody has a common variable immunodeficiency, it's not just a matter of the nuisance of the frequent colds or flus, it actually can predispose us to more severe diseases down the road, so we have to pay attention to it.

The way that one corrects a common variable immunodeficiency is by looking at the five things that are known to suppress the immune system. As I said, poor nutrition, infections, toxins, trauma, stresses. Through blood test and a careful patient history, we narrow down that list, find out what's wrong, correct the nutritional deficiencies that may be there, put the person on detox program if there are toxins that are indicated that would suppress the immune system. Chelate out the heavy metals if that's what's causing the immune suppression, so on and so forth. In more severe situations, we can actually supplement the missing antibodies through the use of immunoglobulin. Immunoglobulin therapy is available both as a subcutaneous form or through the intravenous administration of this substance, which will immediately supply the antibodies that are missing, help to plug the hole temporarily, and buy time for the immune system to rebalance itself.

Everybody has to have an individualized workup and evaluation. When I'm working with people with CVID, each person has their own reasons and their own abnormalities as to why they have it. And each person has to be treated individually to have the maximum and most rapid treatment results.

LET'S EXPLORE DEGENERATIVE JOINT DISEASES

Most of the diseases that I work with have a common denominator of an inflammatory component to that disease or illness. Certainly when we're looking at the situation of degenerative joint diseases, inflammation is a major hallmark of that disorder. Degenerative joint diseases can occur for many different reasons. A common cause is trauma. Overuse and wear and tear over time can literally wear out the joint, so to speak, and we wind up having damage that leads to chronic inflammation, swelling, and pain. Now the kind of situation that we're looking at here is that we're not talking about somebody who's just a marathon runner or an Olympian who wears out a joint because they burn through their cartilage or an NBA basketball player, if you will, who always have problems with their knees and ankles and such that you hear on TV all the time. But even us normal folks, through a process of time, Father Time catching up with us, and us not taking care of ourselves as well as we should, can lead to degenerative joint diseases.

The hallmark is pain, inflammation, and swelling. So there are things that you can do to address these things and stabilize and slow the progression of this disorder, such as exercise. Okay, now I just said that this often occurs as a result of trauma or wearing things out, so exercise might seem a little contradictory, but what I'm talking about when I say exercise is exercise that allows for the normal lubrication of the joint, without having to fight gravity and having the bones bang against each other. So the kind of exercise that I'm referring to is swimming. Swimming is perhaps the best exercise for somebody with a degenerative joint disease cause it allows for the full range of motion of that joint, and in doing so it allows the fluids that have to lubricate that joint to freely float and flow through it without fighting the effects of gravity, without having the bones bang against each other, if you will. So I highly recommend swimming on a regular basis for somebody who has a degenerative joint disease situation.

There's other sorts of things that people can do. There's simple supplements that are often helpful. Things like MSM, chondroitin sulfate for example is another thing that's very commonly helpful. L-glutamine, another supplement that's often helpful. In more severe cases, we have treatments that could be beneficial. Yes, we have anti-inflammatory medicines: Advil, Aspirin, Aleve, etc., can be helpful temporarily to reduce the inflammation and the pain, but they're not gonna be curative. In more advanced situation, there are prescription anti-inflammatories that could be used, but again, they're only going to relieve symptoms and aren't going to be curative.

From a physician's point of view, one of the things that we can do, a therapy that's available to us now is the use of adult stem cells. We can harvest some of your own stem cells from the bone marrow or from your fat tissue, purify it, and then inject it right into the area of the joint that's such affected by the degeneration, and this will stimulate local repair and regeneration of the cartilage to the extent possible to thus relieve the symptoms of the degenerative joint disease. If the cartilage has been completely worn away and there's nothing left, then the stem cells aren't going to have a lot to work with, but if it's a more moderate situation, the stem cell therapy can be almost miraculous in its ability to repair the damaged joint and restore full functioning.

Now, as the joint is repairing itself and healing itself through a process of taking supplements, exercise through swimming, even the use of stem cells, you want to be careful that you don't overuse it and increase the inflammation locally. Again, swimming is to be recommended and when one goes for a walk it should be a walk at a moderate pace, two, two-and-a-half miles an hour, nothing too fast, nothing that's gonna generate a lot of damage or a lot of high impact to the joints in question. You want to wear well-cushioned footwear that allows the absorption of some of the impact when we're walking on firmer surfaces and this allows things to heal at a reasonable pace.

Other things that can be very helpful for degenerative joint diseases are things like massage, physical therapy, and even acupuncture can often be very, very helpful and relieve a lot of the symptoms of degenerative joint diseases. This is a very common disorder. It can occur almost at any age, depending upon the sort of traumas and injuries the person has suffered, and how well their repair mechanisms are working, but there are things you can do about it to help to relieve the symptoms and improve the quality of your life. FACTS ABOUT THYROID DISEASES

If our thyroid is running way below it's functional capacity to help to support our metabolic rate and our metabolism at large, there are many things that can lead to Thyroiditis or Hypothyroidism. One of them is an inflammation of the thyroid called thyroiditis. Thyroiditis can occur as a result of various toxins that can cause damage to the thyroid or various infections that can hit the thyroid, causing a secondary inflammatory immune counterattack that can also lead to collateral damage and an attack of the thyroid tissue itself, and/or attack of the thyroid hormone.

When the thyroid hormones and/or tissue are attacked, it causes a hypothyroid situation because the person may be producing enough of the thyroid hormone but because it's being shoot up by the antibodies you can't get from point A to point B, as efficiently as it should. Then we suffer the symptoms of hypothyroidism. Some of the symptoms may include things such as weight gain, trouble with focus, memory, concentration, sleep disturbance, swelling of the thyroid tissue, and a goiter that can occur around the lower part of the neck.

Some of the symptoms that occur will include a swelling of the thyroid gland, which is located on the lower part of our neck. And sometimes this swelling of the thyroid can be quite dramatic, so that you can see it quite easily just looking at the person. And on the inside, it can cause pressure on the esophagus, and difficulty with some swallowing. It may feel like we have a little lump in our throat or it's hard to swallow certain things because of the swelling of the thyroid tissue.

When we treat somebody with hypothyroidism, the first thing that we do is take a look at the blood test, and see what's going on there. The blood test will include looking at the pituitary hormone, TSH, which has to stimulate the production of the thyroid hormones. We look at the different active thyroid hormones, free T3, and the hormones that are converted to it, free T4, etc., total thyroid hormones. And another hormone of the thyroid produces called reverse T3, which forms a dynamic balance with the active thyroid hormone and helps to regulate its function.

Another part of the evaluation will be to look at the thyroid gland with a safe, simple painless test called a thyroid ultrasound. This takes some pictures of the thyroid with sound waves. And allows us to look at the structure of the thyroid, and see if there are any lumps or bumps there that shouldn't be. Because unfortunately, one of the things that also causes thyroid disease is thyroid cancer. So we wanna be a little bit on the vigilant side to make sure there's not something like that going on in the background, because then it would have to be treated in an entirely different way.

Assuming that there is no sign of any thyroid cancer, and the hypothyroidism is addressed based upon the inflammatory markers that I mentioned earlier, looking for the production and activity of antithyroid antibodies. And thus from there, we work with the immune system to down regulate the production of these antibodies, stabilize the different arms of the immune system, neutralize the inflammatory molecules with things such as essential fatty acids and Vitamin C, Vitamin E, et cetera. And also the use of thyroid hormone.

I like to use different natural thyroid hormones, whether it be Armour Thyroid, Westhroid, Nature-Throid or compound of thyroid because this is a naturally balanced molecule containing both T3 and T4, so the body can recognize it a little bit easier, and it's a little bit gentler on the biochemistry, I find. And it's easy to adjust the dosage by monitoring the levels of thyroid hormone. Also, if a person has an active thyroiditis, an autoimmune disease, then these natural thyroid products are critical because they act on what are called alternate antigens, they're critical in the treatment of the thyroiditis, helping to down regulate the immune system, and shut off the abnormal antibody production.

By putting together a comprehensive individualized treatment strategy, hypothyroidism can be addressed easily by looking at these blood tests and looking at some of the other organs that support and work with the thyroid such as the adrenal glands. Supporting that organ as well, the person can have a remarkably increased quality of life, feeling better, with more energy, better focus and concentration, enjoying their life to the fullest. THE FACTS BEHIND CHRONIC FATIGUE SYNDROME

Chronic Fatigue Syndrome (CFS) occurs in people who are suffering from overwhelming levels of stress for a prolonged period of time. As a result of that stress, there's a progressive weakening of certain endocrine pathways in our system, a weakening of our adrenals, stress in our thyroid gland, so on, and so forth. So ultimately, these organs go out of balance, and out of condition, and out of their normal response range. As soon as there's a shift in the endocrine response, there's a secondary effect on the functioning of our immune system pre-disposing us to infections from various sorts of viruses. There are a number of different viruses that are associated with chronic fatigue syndrome. Research is continuing, but some of them include things like Epstein-Barr virus, Cytomegalovirus, HHV6, Coxsackie B, and others. As I said, the research is continuing.

The common denominator is that the person who is suffering from a situation where their immune system has become confused and damaged, and doesn't protect them the way that it should. Also, their endocrine system, the balance of hormones, is somewhat out of whack in terms of their function of their adrenals and their thyroid. This sort of fatigue that results can be a bone-tired sort of fatigue where the person may get eight, 10, 12 hours of rock solid sleep, wake up in the morning, and feel like they haven't slept for two minutes. This is a sort of fatigue that when they try to go out and do the simplest tasks in their life, to go shopping, to go to work, to take a walk, simple exercise, halfway through, they find they have to sit down, rest or even take a nap. It can be a very, very debilitating disease.


And yet, when you look at the person they may look well, their skin color may be good, the skin tone may be good, lots of nice bushy hair. They may not look sick, but the way that they feel can be completely debilitating. Unfortunately, there's no one blood test for chronicfatigue syndrome, which adds to the problems and the confusion about this disorder. Chronic fatigue syndrome is a syndrome. It's a collection of symptoms. It's not a specific disease, per se. When somebody has chronicfatigue syndrome, the first thing that we do is a differential diagnosis. We'll do a number of different blood tests to see if somebody's suffering from something else that has to be treated very specifically, such as Lyme disease, or some babesia, or from mononucleosis, or from diabetes, or from thyroiditis, and on and on. If we can identify some other distant disease that can account for the symptoms, that then becomes the target, that then becomes their diagnosis until it is resolved and put back into a state of cure or remission, and then, we see what symptoms are left, if any.

If the person doesn't have any known identifiable disease or disorder, then we're left with chronic fatigue syndrome. The way that we address chronic fatigue syndrome is by looking at the balance of the immune system, looking at the inflammatory markers that it is making, looking at the balance of the major pathways of the immune system, looking at the various hormones that the adrenals and thyroid are making, which are responsible for our energy production and stamina, looking at the relationship, looking at the ratios, correcting things as necessary; and then, doing some very specialized blood tests to try to get a better idea of how the mitochondria are working. Mitochondria are like little power houses that are located in each and every one of our cells throughout our entire body.

One of the characteristics of chronic fatigue syndrome when people are doing research on this disorder, is that they find that there is a dysfunction of the mitochondria: A decrease in the number of the mitochondria, and a decrease in their ability to produce the energy molecule called ATP. However, the good news is that research has also been going on on ways to support the mitochondria, to stimulate the production of ATP, and to actually stimulate the production of their number and how to stimulate the production of new mitochondria. So the bottom line is that through a comprehensive evaluation of the person who's suffering from chronicfatigue syndrome, we can put together an individualized treatment protocol that can help to stabilize the symptoms that they're having, and over a period of time, to reverse the syndrome that they're suffering from, so they could have a long, healthy, and happy life.

CHRONIC FATIGUE SYNDROME: THE HIDDEN EPIDEMIC

WHAT IS COPD?

Chronic Obstructive Pulmonary Disease or COPD occurs as a result of progressive damage, inflammation and scarring to the lungs. The lungs, unlike the liver, don't repair themselves through a process of regeneration. Lung tissue repairs itself through a process of scar tissue formation. So if somebody's exposed to various toxins long enough that are toxic to lung tissue, the lung cells can begin to die, and as such, they will begin to cause the production of scar tissue. Over a period of months and years, the scar tissue will accumulate, and the person's lung function, their pulmonary function will gradually begin to decrease and diminish. Their ability to exchange carbon dioxide for oxygen will decrease and diminish. As a result of this, and then the lower levels of oxygen in their blood that occur over time, the person will suffer from things such as, more easily fatigueability, decreased energy, decreased stamina, decreased memory, focus, concentration, and of course, shortness of breath. COPD can be a very debilitating sort of illness, and in severe forms, people wind up in a wheelchair, on oxygen. 

Many people have contracted COPD as a result of exposure, for example, to the toxins from 9/11. Many of the healthcare workers and the emergency personnel that worked there for the days and weeks following, who were inhaling some of these toxins, wound up with a World Trade Center lung syndrome, and as result, they wound up with some permanent scarring and damage to their lungs and a certain degree of COPD. More common forms of COPD occur as a result of smoking or the exposure to other sorts of industrial toxins and fumes. Back in the old days, when we still had lead in our gasoline, people that worked for the Triborough Bridge Association, and worked in the tunnels, for example, and were inhaling these sorts of fumes all day long, commonly developed various sorts of lung diseases. People that work in mines, coal miners or other sorts of miners, develop forms of COPD as a result of inhaling these sorts of dusts and toxins on a daily basis.

COPD can also occur as a result of infection. There's a sort of infection that can occur from different types of fungi. Farmer's lung, for example, is one such example of people who are working with straw and hay, and in an enclosed environment where mold can be growing in the hay. Breathing in the mold spores could actually cause an infection in their lungs. And as a result of that infection, you have damage to lung tissue and scar tissue formation. Other sorts of infections can also lead to the formation of lung damage and scar tissue formation. Now when the lungs are scarred, as I said earlier, you're kinda stuck with that. But along with the scar tissue formation, there's also typically an increased level of inflammation. With the inflammation comes an increased production of mucus. So these things are things that we can intervene and help with. We can reduce the inflammation by rebalancing the immune system. We can reduce the mucus that forms, that can clog the small airways, through the use of different types of mucolytic agents, whether it be natural mucolytic agents, or whether it be pharmaceutical ones. There's a wide range of things that we can use to help to liquefy the mucus, get it out of the person's system, so they could breathe easier. Also, exercise over a period of time helps to stimulate other parts of the lungs that haven't been such affected to compensate to a certain extent and improve overall pulmonary function.

One of the better exercises is swimming, for example, because it allows for lots and lots of deep breathing. As you're doing the overhand crawl, you're physically and mechanically expanding the chest cavity, breathing in lots of good air presumably, and thus increasing the oxygen levels in your blood, or exercising all the accessory muscles in your ribcage that help the lungs to expand and contract. Other things that could be done to help with people with COPD, are in the early and somewhat experimental stages with the use of adult stem cells. There's research that shows that if we take our own stem cells that we can get from the bone marrow or from our fat tissue, concentrate them, purify them, and inject them intravenously, that this too can help with pulmonary function to some extent.

When we inject stem cells intravenously, they tend to migrate to areas of chronic inflammation. So first, of course, we'll take a look at the person's blood test to see what their systemic levels of inflammation are, address them as much as possible. So the only information that's really left is that in the lungs where you want the stem cells to go. And then when we infuse them intravenously, they tend to migrate to the areas of lung damage and can, over a period of months, gradually improve pulmonary function. This kind of treatment is still relatively early in our therapeutic armamentarium, but it shows great promise for people with COPD. The bottom line is, the sooner that you address the COPD, the less further lung damage will accumulate, and the better the quality and quantity of life will be.
